SELF-SERVICE CHECK IN

Our Self-service Check-in is now available to use on arrival at the clinic. We ask that you carefully check you name/address/phone details as well as you Medicare card and any concession cards you may have to ensure all details are correct.

This service is only available to existing patients.

All new patients attending the clinic for the first time must report to reception staff on arrival.

As of the 1st September 2016 our Gilbert street clinic will be closed. Dr Puja Mahajan is now consulting at Cornwall St and all services formerly provided by our clinic at Gilbert St are still available at Cornwall St. For all appointments and enquiries please phone our Cornwall St Clinic on 8582 2855.

FLU CLINIC

Free Flu Vaccine Criteria:-

– Everyone over 65 years of age.
– Pregnant women.
– Suffer from an underlying chronic medical condition e.g. Cancer, Asthma, Diabetes, Heart conditions.
– Aboriginal or Torres Strait Islander children aged 6months up to 5 years of age.
– Aboriginal or Torres Strait Islander people 15 years of age and over.

If you do not meet the free criteria you will be asked to pay for your vaccine.
Prices to be set annually
This service requires a standard doctor’s appointment.

CHANGES TO MEDICARE

From 1st of July 2016, Medicare patient cheques are stopping. You will need to make sure your bank account details are registered with Medicare to get your benefit.

There are a number of ways you can register your bank account details with Medicare:

  1. Use you Medicare online account through myGov or the Express PLUS Medicare mobile app.
  2. Complete a Medicare bank account details collection form and post it to Medicare.
  3. Call Medicare on 132 011 and tell them your bank details.
  4. Lodge a completed Medicare bank account details collection form at your Medicare office.

 

Whether you claim at the surgery, online or in person, once your bank account details are registered, your Medicare benefit will be paid directly into your account.
